Bridging Policy and Practice: Advancing Equity through Social Determinants of Health

This session will examine how social determinants of health (SDOH)—such as housing, education, food security, and economic opportunity—shape client outcomes and community well-being. Social workers are uniquely positioned at the interface of policy and practice, addressing immediate client needs while advocating for systemic changes that reduce inequities. By integrating SDOH into assessment, intervention, and policy advocacy, social workers can strengthen their role as change agents, ensuring that practice not only responds to individual challenges but also influences the broader structures that create them. Participants will gain practical strategies to embed SDOH in their daily work and to use advocacy as a tool for advancing equity at local, state, and national levels.

About the Presenter
Neerja Singh is the Area Manager for Children’s Mental Health in Hennepin County, MN and a 2023 Bush Fellow. With more than 20 years of leadership experience in behavioral health and human services, she has also served as Deputy Director and Behavioral Health Clinical Director at the Minnesota Department of Human Services. Neerja teaches graduate social work at the University of St. Thomas and serves on the NASW-MN Ethics Committee, where she helps lead statewide ethics consultation circles. Her work centers on bridging policy, practice, and advocacy to advance equity, community voice, and culturally responsive systems of care.
